Participation agreement 2026–27
DRAFT FOR LEGAL REVIEW. This text has not been reviewed by a lawyer and must not be relied on until TrailBreak publishes a reviewed version.
1. The activities. TrailBreak programs (Adventure Pods, Afterschool and Mentorship) take place outdoors in the Wasatch Front: trails, canyons, creeks, parks and other natural areas. Activities can include hiking, scrambling on rocks, crossing or playing near water, using hand tools such as knives for whittling, building shelters and fires under supervision, and spending time in changing weather.
2. Risks. Outdoor activities carry risks that cannot be eliminated, including slips, falls, cuts, bruises, sprains, broken bones, insect stings and bites, allergic reactions, exposure to heat, cold, sun or lightning, getting lost or separated, contact with plants and wildlife, and travel to and from activity sites. Serious injury is possible.
3. Our part. TrailBreak guides plan routes for the age group, check weather, carry first-aid kits, follow the care information you give us (allergies, medications, conditions), keep a roster at check-in and check-out, and only release your child to people on your approved pickup list.
4. Your part. You confirm that your child is able to take part in active outdoor programs, that the medical, allergy and contact information you give us is complete and kept up to date in the family portal, and that your child will bring the gear on the program's gear list. You agree to tell us about changes that affect your child's safety.
5. Medical care. If your child needs medical attention, TrailBreak staff may give first aid, give medications you have authorized (including an epinephrine auto-injector), call emergency services, and arrange transport to a medical facility. We will contact you as soon as possible. You are responsible for medical costs.
6. Release and assumption of risk. [For the lawyer: release, waiver and indemnity language appropriate under Utah law, including the limits on releasing claims on behalf of minors.]
7. Photos. Photo and media consent is a separate form. Signing this agreement does not give photo consent.
8. Changes and cancellations. Our refund and cancellation terms are shown at checkout and in Billing.
By signing, you confirm you are the child's parent or legal guardian, that you have read this agreement, and that you agree to it.
